495N to exit 19 and you're practically there. That's route 109. Follow route 109 east for a little over 5 miles. It twists and turns a little, but as long as you stay on 109 you'll drive right up to Ned's. The Yahoo Maps directions are a little more specific.

9. Take exit #19/RT-109 toward MEDWAY - go 0.3 mi
10. Turn Left on MEDWAY ST[RT-109] - go 0.4 mi
11. Continue to follow RT-109 - go 4.9 mi
12. Arrive at 1590 MAIN ST, MILLIS, on the Right

basically from the Cape
there are 2 ways

Route 3 North
Route 128 towards Dedham (bear left at the Braintree split)
Route 109 West towards Westwood
...we are 12 miles on the left (Millis-Medway line)

Route 495 North
Route 109 East towards Medway
...we are 6 miles on the right (Millis-Medway line)
