5.8-6.0 ph

that is a little lower than I like

I would discontinue filtering with carbon

if you are using a Magnum
you can put different stuff in the Carbon retainer sleeve

foam blocks, gravel, ceramic ring media , etc.

also maybe continue the vitamins
change water more often
maybe dose with metronidazole

check for electric charge in the water
(frayed heater cord, etc.)

What are you feeding ?
(While I agree with Ned that its usually carbon or poor water quality, I've seen this from poor diet as well)

i use cichlid pellets from hikari and they also eat some shrimp pellets that i put in for my bottom dweller fish . i also put some bloodworms for my loaches once in a wile.they seem to like those as well. and i put algae tablets in there for the plecos. and once aday i put some flake food in there. the cichlids try to eat everything i put in there but i try to feed them first so they dont take from the others
